Certain types of entry by OHS access card holders require notice in writing 24 hours prior to the intended entry. The following form has been developed for this purpose. The form is not mandatory and any form of written notification is acceptable provided that the requirements of Section 22G of the Workplace Health and Safety Act 1995 are fulfilled.
