Notification to customer and recording of electrical work
The approved form to be given to the customer by a person responsible for electrical work as required by Regulation 17 (2) of the Occupational Licensing Regulations 2008 is either,
(a) A correctly completed copy of the Electrical Installation Notice (EIN)
or
(b) A document (some of which may be pre-printed) that contains as a minimum the following
- Electrical Contractor’s name
- Electrical Contractor’s licence number
- Electrical Contractor’s address
- Customer’s name
- Customer’s address
- Details of the electrical work performed must

- Be sufficient to identify what work was carried out, when and where, and
- Indicate the part of the building where the work was done

- Use phrases that relate to the location within the building
- Avoid names of people’s rooms
- A duly completed certification statement that says:

- I certify that I have systems in place to ensure that the above mentioned work has been carried out in accordance with the installation and testing requirements of AS/NZS 3000 or ENA C(b) 1 and that I have recorded and retained (as required) the documentation prepared by the licensed electrician who certified this work as compliant.
- Nominated Managers name ..........................
- Signed (electronic signature permitted)....... (date).......
- As the customer and recipient of the abovementioned work you should retain this document as your record of the work undertaken.
Additional requirements (not for inclusion on customer’s copy)
A condition of both options (a) and (b) above is that for the purposes of Regulation 17(1)
- the licensed electrician responsible for the electrical work certifies the work as compliant and that certification is recorded.
- the name of the electrician responsible for the certification is clearly identified on the same record.
- all appropriate tests have been undertaken in accordance with the Standard of Work Code of Practice (AS/NZS 3000 and/or ENA C(b)1) and recorded.
- the electrical contractor (or in the absence of an electrical contractor, the electrician) retains the above records for 10 years.
