7. Lost, Damaged or Destroyed Licence

If your Licence to Perform High Risk Work is lost, damaged or destroyed you can apply for a replacement licence.

You will be required to

  • pay a fee
  • complete the application form including your licence number 
  • complete a statutory declaration stating the reason for applying for a replacement licence. Workplace Standards will use your file photograph.
