8. How to lodge your application
- To lodge your application, take it in person to a Service Tasmania shop.
- You must ensure that your application form is completed correctly and you have all supporting documentation required (See Application Checklist below).
- If you are applying to transfer endorsements on a current Certificate of Competency to a Licence to Perform High Risk Work, or if you are applying to add an additional class or classes to an existing Licence to Perform High Risk Work previously issued - you are required to attach your certificate(s) / licence to your application form.
- The Service Tasmania Officer will check your application form and supporting documentation, verify that you are the person in the application by acknowledging your evidence of identity, take your photograph (unless you are applying for a replacement licence), witness your signature, accept your application fee and provide you with a receipt.
- Service Tasmania will forward your application to us. You will receive your licence in the mail at the postal address you supplied (Part B).
- Do not sign the form. You are required to sign your application in front of a Service Tasmania officer at the time of lodgement.

Please note: applications not completed correctly or not accompanied by the required documentation will not be accepted.
