8. How to lodge your application

  1. To lodge your application, take it in person to a Service Tasmania shop.
  2. You must ensure that your application form is completed correctly and you have all supporting documentation required (See Application Checklist below).
  3. If you are applying to transfer endorsements on a current Certificate of Competency to a Licence to Perform High Risk Work, or if you are applying to add an additional class or classes to an existing Licence to Perform High Risk Work previously issued - you are required to attach your certificate(s) / licence to your application form.
  4. The Service Tasmania Officer will check your application form and supporting documentation, verify that you are the person in the application by acknowledging your evidence of identity, take your photograph (unless you are applying for a replacement licence), witness your signature, accept your application fee and provide you with a receipt.
  5. Service Tasmania will forward your application to us. You will receive your licence in the mail at the postal address you supplied (Part B).
  6. Do not sign the form. You are required to sign your application in front of a Service Tasmania officer at the time of lodgement.

Please note: applications not completed correctly or not accompanied by the required documentation will not be accepted.

Application Checklist

What you need to take with you to the Service Tasmania shop

  • Your completed application form and the required application fee (Located on Resources page linked below).
  • Your Current Licence to Perform High Risk Work card (for an additional class or interstate transfer only). All current issued certificates if applicable. These will be attached to your application for processing.
  • Evidence of Identity documents (these documents will be returned to you upon verification by Service Tasmania). This is only required for new applicants (who do not hold a Licence to Perform High Risk Work). See Evidence of Identity Requirements below.
  • If you do not receive your new licence within 30 days of the application you will need to contact Workplace Standards Helpline on 1300 366 322. If you do not contact Workplace Standards you may need to attend Service Tasmania to reapply and a new fee will be incurred.
