Replacement cards

If you have lost, damaged or had your ID card stolen, you are required to inform Workplace Standards Tasmania on 1300 366 322 (within Tasmania) or (03) 6233 7657 (outside Tasmania).

Tasmanian Resident Applicants

For a replacement ID card you will need to

• Complete a Statutory Declaration form stating the reason for replacement
• Lodge the form at any Service Tasmania shop
• Fee is $35.00 (from 1 July 2011)

Statutory Declaration Forms are available from Service Tasmania shops or via this link Justices of the Peace and Commissioners of Declarations 

Interstate Applicants

• Complete Statutory Declaration
• Attach a certified passport photo and copy of driver’s licence
• Forward to Workplace Standards Tasmania, PO Box 56, Rosny Park, Tasmania 7018
