Change of address
Notification of your change of address for licences/permits/registrations or certificates listed and issued by Workplace Standards Tasmania can be achieved by one of the options below.
Type of licence/permit/registration/certificate
- high risk work licence
- building practitioner accreditation
- practitioner's (electrical work) licence
- electrical contractor's licence
- plumber’s licence
- gasfitting licence
- automotive gasfitter
- dangerous goods bulk driver’s licence
- shotfirer permit
- medical practitioner accreditation for workers compensation
Options for advising us of changes
- complete the online Change of Address form
- download the Change of Address Form word document, complete and return by fax to (03) 6233 8338
- send an email stating your full name, date of birth, old address, new address, type of licence and licence number to wstinfo@justice.tas.gov.au
- written notification with the same details as above to PO Box 56, Rosny Park TAS, 7018
- contact the Helpline to send you a Change of Address Form (GF010)
