Change of address

Notification of your change of address for licences/permits/registrations or certificates listed and issued by Workplace Standards Tasmania can be achieved by one of the options below.

Type of licence/permit/registration/certificate

• high risk work licence
• building practitioner accreditation
• practitioner's (electrical work) licence
• electrical contractor's licence
• plumber’s licence
• gasfitting licence
• automotive gasfitter
• dangerous goods bulk driver’s licence
• shotfirer permit
• medical practitioner accreditation for workers compensation

Options for advising us of changes

• complete the online Change of Address form
• download the Change of Address Form word document, complete and return by fax to (03) 6233 8338 
• send an email stating your full name, date of birth, old address, new address, type of licence and licence number to wstinfo@justice.tas.gov.au
• written notification with the same details as above to PO Box 56, Rosny Park TAS, 7018
• contact the Helpline to send you a Change of Address Form (GF010)
