Change of address

Notification of your change of address for licences/permits/registrations/certificates or magazine subscriptions listed and issued or supplied by Workplace Standards Tasmania can be achieved by one of the options below.

Type of licence/permit/registration/certificate or subscription

• high risk work licence
• accredited building practitioner
• construction induction (white card) 
• practitioner's licence (under Occupational Licensing)
• contractor's licence (under Occupational Licensing)
• automotive gasfitter
• dangerous goods drivers' licence
• dangerous goods vehicle licence
• registered assessors 
• shot-firer permit
• medical practitioner accreditation for workers compensation
• CONNECTIONS and Workplace Issues Magazine

Options for advising us of changes

• complete the online Change of Address form
• download the Change of Address Form word document (GF010), complete and return by fax to (03) 6233 8338 
• send an email stating your full name, date of birth, old address, new address, type of licence and licence number to wstinfo@justice.tas.gov.au
• written notification with the same details as above to PO Box 56, Rosny Park TAS, 7018
• contact the Helpline to send you a Change of Address Form (GF010)
